Diagnostic Laparoscopy through the Hernia ring for Incarcerated Hernia


Abstract
When operating the incarcerated or strangulated inguinal hernia, if there is no evidence of intestinal ischemia, most surgeons proceed with hernia repair without attempting to visualize the previously incarcerated bowel. however, when there is
evidence
of intestinal ischemia, an attempt should be made to locate the previously incarcerated bowel through the sac in order to ascertain its viability. Should this not be successful, they recommend exploration through an abdominal incision. For
reducing
the
change of abdominal exploration through a separate incision, we have devised a new mehtod (93/12/10) using laparoscopic instrument. By inserting the trocar and telescope through the internal inguinal ring, we can explore the viability of the
bowel
and
confirm the presence of the counter side hernia. By doing this we can get another information in the abdominal cavity also. Because it dose not need another incision or opening, its complication is minimal and the effect is very good.
